01 Incident Log

Report it in a minute.Fix it before it hurts.

Most hazards are seen long before they injure anyone, by an operator, a contractor or a forklift driver. The Incident Log gives every one of them a one-minute way to report what they saw, with a photo, in their own language, with or without their name.

How it works

Three steps on the shop floor.

  1. 01

    Scan and report

A worker scans the QR code at the gate, bay or machine, picks the event type, adds a photo and taps submit. No login needed for reporters.

  2. 02

    Triage and investigate

The safety officer gets the report instantly, sets severity, and runs a 5-Why or fishbone investigation for anything serious.

  3. 03

    Act and close the loop

Root causes become CAPAs with owners. The reporter is told what changed, which is what keeps people reporting.

What Incident Log does.

Any phone, no app

Reports open in the phone browser from a QR code. Works on basic Android phones and slow plant Wi-Fi.

Hindi, Marathi, English

The form, categories and prompts appear in the reporter's language. The safety team reads them in theirs.

Anonymous when needed

Reporters can leave their name off. Anonymous reports are handled with the same priority as named ones.

Photos and location

Up to five photos per report, tagged to a plant, area and equipment so patterns show up by location.

Serious event escalation

Lost-time injuries and dangerous occurrences alert the plant head and EHS head at once, with a statutory reporting checklist.

Built-in investigation

5-Why and fishbone templates, witness statements and a timeline, all attached to the event record.

Every type of event, one log

The log separates events so your numbers mean something. Near-misses are not mixed with injuries, and first-aid cases are counted on their own.

  • Near-miss: something that could have hurt someone
  • Unsafe act and unsafe condition
  • First-aid case and medical treatment case
  • Lost-time injury and dangerous occurrence
  • Environmental release, spill or leak
  • Fire, property damage and vehicle incident

Designed for the people who see hazards first

Contractors, loaders and new joiners are often closest to the hazard and least likely to speak up. The log is built around them.

  • Large tap targets that work with gloves
  • Icons and photos instead of long text
  • Contractor and visitor reporting from the gate QR
  • Reporter receives an update when the fix is done

Questions

About Incident Log.

Do workers need an account to report?
No. Reporters scan a QR code and submit. Only safety officers, supervisors and action owners have accounts.
Can we report an incident that happened offline?
Yes. If the network drops, the report is saved on the phone and sent as soon as the connection returns.
How are serious injuries handled?
Lost-time injuries and dangerous occurrences trigger an immediate alert to the plant head and EHS head, and open a checklist for statutory notice to the authority. The plant decides and files the notice.
