02 CAPA Tracker

No action left open.No closure without proof.

A near-miss report is only worth something if the hazard is removed. The CAPA Tracker gives every corrective and preventive action an owner, a deadline and a closure photo, and it climbs the ladder on its own when a deadline slips.

How it works

Three steps on the shop floor.

  1. 01

    Raise from anywhere

CAPAs are raised from an incident, an audit finding, an AI Hazard Eye alert, a legal requirement or a management review.

  2. 02

    Assign and chase

Each action gets one owner and one deadline. Reminders go out before the due date, and overdue items escalate to the department head and then the plant head.

  3. 03

    Prove and verify

The owner closes with a photo or document. The safety officer verifies, and an effectiveness check is scheduled for 30 or 90 days later.

What CAPA Tracker does.

Corrective and preventive

Separate the quick fix from the change that stops it happening again in other bays and other plants.

Closure proof

A before and after photo, a work order or a revised SOP is required to close. No proof, no green lamp.

Escalation ladder

Owner, then department head, then plant head. Each step is logged so nobody is surprised in the monthly review.

Hierarchy of controls

Every action is tagged elimination, substitution, engineering, administrative or PPE, so you can see how often you rely on PPE alone.

Effectiveness checks

A follow-up check confirms the fix still works weeks later, and reopens the CAPA if it does not.

Spread the lesson

Copy a CAPA to other plants with the same machine or layout, and track each copy to closure.

What the plant head sees

Open, overdue and closed actions by department, by source and by owner, with the oldest overdue items at the top.

  • Overdue CAPAs by department and owner
  • Average days to close, by severity
  • Share of actions that rely only on PPE
  • Actions reopened after an effectiveness check

Questions

About CAPA Tracker.

Can one finding have more than one action?
Yes. A finding can have several actions with different owners and deadlines. The finding closes only when every action is closed and verified.
Can action owners work from a phone?
Yes. Owners see their list on the phone, upload closure photos from the job and get reminders by email.
Can we change the escalation timings?
Yes. Reminder and escalation timings are set per severity, for example two days for high-risk actions and seven days for low-risk ones.
